Centers Plan for Healthy Living (CPHL), is a Managed Care Organization servicing members with Medicare and/or Medicaid.  Our goal is to provide members and all those involved in their care with the guidance and health plan choices they need for healthy living. CPHL is committed to providing quality, coordinated health care to some of the most honored and yet still vulnerable members of our community.




Oversees, coordinates, plans and organizes the day-to-day operations and administrative matters to support the goals and objectives of the Provider Relations department. Assists in the coordination and management of administrative matters and handles confidential information. Ensures efficient and accurate provider lead distribution to internal and external Network Development resources. Works under the direction of Provider Services Supervisor. 



  • Daily management  of Casetrakker Provider Issue Log
  • Assignment of Casetrakker Provider Recruitment Requests
  • Follow up with Providers on recruitment requests
  • Preparation of Recruitment Packets
  • Scheduling Site visit appointments
  • Updating records in Casetrakker



Required: High School Diploma or the equivalent required

Preferred: Bachelors Degree in Business Administration, Health Care Administration or other related field, or the equivalent preferred.


Type of Experience                                         

Required:   Minimum of three years progressive administrative, business or office experience required.


Specific Technical Skills

Required:    Advanced personal computer skills, including Microsoft Word, Excel, Access and Outlook required. Effective oral, written communication and interpersonal skills required. Ability to multi task in a fast-paced environment required.
