JOB SUMMARY: 

The Pharmacist gives pharmacological information to multidisciplinary health care team regarding CPHL member drug therapies.

 

 

Responsibilities

PRIMARY RESPONSIBILITIES:     

  • Works with Care Managers to complete medication reconciliations and make recommendations.
  • Reaches out to MD’s to have them prescribe as per best practice medical guidelines.
  • Educates RN’s on newest therapies and their uses.
  • Medication Review of difficult cases.
  • Participates in Care Plan meetings and makes recommendations for best care for plan members.
  • Assures compliance with regulatory agencies as it pertains to Pharmacy Department operations
  • Collects and analyzes data to assess effectiveness of pharmacy interventions
  • Review and take appropriate action of Pharmacy Reports

 

Qualifications

Education and Experience:

Required:

  • Licensed Pharmacist in the State of New York
  • Managed Care Experience, particularly with oversight of Pharmacy Benefits (PBM Operations)

 

 

Specific Technical Skills:

Required:

Proficiency with Microsoft applications (Word, Excel, PowerPoint, Outlook)
