JOB SUMMARY:

 

Provides and supports the educational program for the CPHL clinical team. Evaluates and creates an infrastructure to support high quality clinical coordination and service through policy, procedure, and practice guideline integration in conjunction with clinical nursing leadership. Responsible for program wide oversight and evaluation of clinical development and training of CPHL clinical staff and CPHL strategic partners. 

   

Responsibilities

PRIMARY RESPONSIBILITIES: 

 

  • Leads the development, implementation, and evidence-based, quality education programs and activities which promote individual development, competency and delivery of high quality clinical service delivery.
  • Collaborates with management, staff and external partners to assess, integrate, and evaluate educational needs and resources supporting CPHL goals.
  • Develop, review and update departmental training manual, policies/procedures, related job aides and clinical pathways.
  • Educate staff in the clinical policies criteria and procedures of MLTC Care Management and UAS-NY Assessment, as well as an overview of the department, general functions and responsibilities, department relationships with other departments.
  • Conduct new staff orientation, review of care management activities and documentation.
  • Participate in development, testing and implementation of new programs and systems.
  •  Responsible for planning, developing, evaluating and implementing educational programs and training activities based on assessed learning needs of the MLTC/Strategic clinical team.
  • Conduct quality review and audits of clinical team and strategic partners to ensure CPHL protocols and standards are being met.
  •  Performs other duties as assigned or required.

 

Qualifications

Education and Experience:

 

Education

  • Graduate of an accredited four-year nursing program (advanced degree preferred)·  
  • Registered Nurse with minimum five years clinical experience and a current New York State license                   
  • Bachelor’s Degree in Healthcare or related field, Masters preferred
  • Minimum of  5 years of managed care experience working with dual eligible populations
  • Previous managed care and case management experience
  • Minimum of at least 3 years’ experience in development of clinical guidelines and training of clinical staff (nurses and social workers) on care management protocols

Certifications/Licensure:

Required:    

NY State RN License            

 

Knowledge and Skills:

  • Strong problem-solving skills
  • Excellent written and oral communication skills
  • PC literacy
  • Knowledge of current community health practices for the frail adult population and cognitively impaired seniors and disabled
  • Understanding of and sensitivity to cultural differences and needs of the community are essential.
  • Must be adept at operating within a diverse and multi-cultural work environment and community of members such as Spanish, Chinese, Russian, Creole and Korean
