    Overview:

     

    The position is responsible for leading the social work clinical staff with the goal of providing exceptional social work service to the MLTC/FIDA members. This is accomplished through active involvement and leading the day-to-day operations of a social work team and ensuring staff is consistent with CPHL policies and procedures and are compliant with contractual, state and federal guidelines. Serves as a social work resource to members, providers, and care team staff as well as Care Management leadership.

     

     

     

     

    Responsibilities

    Responsibilities:

     

    • Responsible for effective communication and administration of company policies and procedures and ensuring compliance with these policies, as well as State and Federal regulations
    • Supervises, analyzes and coordinates the daily activities of social work practice to ensure departmental productive goals are met with regards to quality timeliness, accuracy and consistency of the formation of intervention goals and care planning.
    • Develops and monitors goals for staff and provides ongoing feedback and coaching.  Participates in hiring process. Provides clinical supervision, support to and oversight of the social work staff. Conducts performance reviews.
    • Continuously evaluates the MLTC/FIDA psychosocial assessment and person-centered care planning integrated team model to improve processes.
    • Participates in team integrated case round meetings with clinical staff.
    • Represents the Plan with external customers, providers and agencies.
    • Coordinates data collection, reviews compliance reports and identifies opportunities for service improvements.
    • Recommends, develops and implements department policies and procedures and interfaces with other areas of the care team to insure consistent application and support.
    • Ensures staff meets all regulatory requirements and comprehends and complies with best practices, professional standards, internal policies, and procedures.
    • Completes regular member records audits to assess the quality of documentation and provides feedback to staff members on areas for improvement.
    • Performs oversight visits in the field with social workers for observation and ongoing support and education.
    • Troubleshoots any issues that social workers are having during field visits.

     

     

     

     

     

    Qualifications

    Requirements/Experience

    • Master’s Degree in Social Work
    • LCSW
    • Minimum two years management experience
    • Five years social work experience working in health care setting.
    • MLTC or geriatric experience preferred.
    • Computer skills that include knowledge of Microsoft office suite of products.
    • Exceptional interpersonal and leadership skills
    • Knowledge of applicable state and federal guidelines
