    The diseases management educator, educates members on the necessary lifestyle changes involved in their diagnosis, including disease management, preventive screening, nutrition, medication, physical activity and testing. 




    • Assessment of clinical status and treatment needs, monitoring of condition and progress
    • Development and implementation of case action plans based upon clinical needs assessment.
    • Collaborative development of alternative plans of care with providers
    • Effective communication with patients, providers, and clients, as well as internal staff. Includes education of patients in an advocacy role.
    • Evaluation and documentation of disease management impact; revision of plans to increase effectiveness. Research and referral to community resources when appropriate or not funded by insurance
    •  Accurate, timely and complete documentation in assessment reports, database notes, and other forms of statistical recordkeeping
    • Maintenance of clinical knowledge base; active pursuit of continuing education
    • Cross coverage for other positions in department as needed
    • Compliance with all verbal and written policies and procedures
    •  Other duties as assigned




    Education and Experience:


    Education and specific Technical Skills


    • BSN, Certified Educator, Master’s Degree Preferred
    • Must have in-depth knowledge of biology and social sciences and be skilled in communication and counseling.
    • Proficient in Microsoft Word, Excel, and Outlook




    • ·NYS RN License or BC-ADM, Certification from the National Commission for Health Education Credentialing (NCHEC),
    • 3-5 years of experience as a disease management Educator
    • At least 2 years of experience as a home care nurse.
